My D tool projects have moved

I moved DCD, D-Scanner, dfmt, and other D tool projects to the 
dlang-community organization on Github:

This should make things more convenient if I get hit by a bus, 
decide that Malbolge* is the one true programming language, or 
just take too long to review a pull request.
