A declaration being inside the *same* file is *also* part of the 
file/directory structure. I could also be in different files. The point 
is that if this kind of knowledge is encoded in the documentation, or it 
is tied to that particular structure. This is the case for the Phobos 
documentation and is the reason why the DDOX based docs have to 
completely ignore the manual cross references.
