I no longer maintain fax or land-line phone capabilities either.  I 
will probably drop by Kinko's or see if I can get the University to 
send it for me (yes, they charge me for personal faxes ... even more 
than Kinko's did at last check).

It appears that the fax number to which the form is intended to be 
sent is shared by Dale and Sherry (contact information on page 
1).  You might inquire about alternate methods for submission.
