Ddoc outputs seemingly valid HTML, except it does not include a DOCTYPE
declaration.

It should output something like the following at the beginning of every
generated file:

<!DOCTYPE HTML PUBLIC "-//W3C//DTD HTML 4.01//EN"
   "http://www.w3.org/TR/html4/strict.dtd">
