Ready to make page-per-item ddocs the default?

std.algorithm has many of the "descriptions" showing samples. Also, I 
know the table at the top is to make things easier for standard ddoc, 
should that be removed?

BTW, I'm all for the docs to be switched. Just the cross-referencing 
alone is worth it.
