Have you thought of simply postprocessing REF_ALTTEXT for your 
documentation engine for now?
That won't lead to merge conflicts ;-)
e.g.

sed -i "s/[$](REF_ALTTEXT \([^,]*\),\([^,]*\),[^)]*)/[\2|\1]" 
**/*.d
