Rename 'D' to 'D++'

According to Wikipedia, D was influenced by:

C, C++, C#, Eiffel, Java, Python (English version)
C, C++, Java, C#, Python, Ruby (Spanish and German version)

I suggest to call it "DythonubyavaC#++" henceforth.
