Rename 'D' to 'D++'

You dont have to get all salty about it, just admit that D is 
more like C++ and then we can propose the name change officially 
on github. Maybe this would help with adoption too, Rust has no 
problem calling itself a successor to C++ rather than C, so D 
shouldnt be afraid of this legacy either.
